Ask a questionTo benefit from primary medical care paid from the compulsory health insurance funds, every citizen must register with a family doctor at a primary care medical institution.
You can choose only one primary care medical institution, preferably the one closest to your place of living.
Changing the family doctor or the primary care medical institution can be done once a year from the last registration or change, except in the following situations:
- change of home address to another locality or sector;
- registration of pregnant women;
- termination of the family doctor's activity at the medical institution.

How to apply
- Check availability
Contact the family doctor you have chosen to obtain their agreement to enrol you on their patient list.
- Submit the application
Complete the application according to the preset model and submit it to the family doctor.
The application is submitted if:
- you are registering for the first time;
- you are changing the family doctor within the same medical institution;
- you opt for a family doctor from another medical institution.
- Confirmation of registration
The application is accepted and confirmed by applying the family doctor's handwritten signature and stamp, or by electronic signature.
- Check the registration
You can check which family doctor you are registered with:
- online, in the Personal Cabinet or the EVO app;
- the online service for checking the compulsory health insurance status.

Form no.1-33/c
the standard form (Form no.1-33/c) completed in writing by the person, by which they express their wish to register with the medical-sanitary institution that provides primary medical care, with the family doctor of that institution
The child's birth certificate
in the cases provided for by the legislation, presented in addition, depending on the activity carried out by the applicant
